We are so excited to have you coming for your assessment at Nostos! There are a few things we would like you to know before you arrive, to make your visit run as smoothly as possible.

• Please try to arrive on time for your appointment. Before you meet with the doctor we will have health history forms for you to fill-out. Should you arrive late, it may cut into the time allotted for your appointment.

• Please bring your Alberta Health Care card (or other provincial health number) with you.

• All practice members under the age of 18 are required to have a parent or guardian present for their appointment.

• Please remove your outdoor footwear at the front door of the clinic. Remember to bring a change of socks or slippers if you're not comfortable being barefoot.

• It's best to wear comfortable clothing that is easy to move in.

• The doctor will spend time during your appointment to complete an examination, review your concerns and build a treatment plan specifically for you. This may include a combination of different therapies. You will have an opportunity to discuss the plan and the options.

• We ask that you bring all of your belongings into the treatment room with you. This keeps our shared spaces clear for other practice members.

• Please be advised that we do not offer direct billing at the clinic. All practice members will receive a receipt after their visit to submit on their own behalf to their insurance plan.

• A $50 missed appointment fee will be applied to your file in the event that you miss your appointment without providing any notice.
